Please submit your completed application form by fax or mail. We will acknowledge all successful applicants within 2-3 business days via telephone call or an email.

Please remember to provide all of the information that the application form requires, including:

- a recent photo,
- copy of birth certificate or copy of passport,
- most recent report card,
- the application fee of $200 CDN

Missing information will delay the process.
